The purpose of the Intake and Eligibility Specialist is to identify the patients needs and provide linkage to services at Northern Nevada HOPES. The Intake and Eligibility Specialist engages patients through the initial intake by completing a needs assessment, orienting them to the processes at HOPES, linking them to services and explaining payment options and helping them enroll in Medicaid and Medicare services. On a typical day, an Intake and Eligibility Specialist may speak with patients seeking appointments, schedule new patient appointments, explain resource availability to patients, assist patients in obtaining state, county and local program assistance, and educate patients on benefits and services. The Intake and Eligibility Specialist receives a warm hand-off from the Service Excellence Navigator in optimizing the patient experience and continues the excellent first impression initiated by the Service Excellence Navigator.

  • We are a Federally Qualified Health Center (FQHC). The mission of all FQHCs is to provide excellent health care services to otherwise under-served populations. We accept most insurance plans, Medicaid, Medicare, and the uninsured. For the uninsured, we offer discounted services based on a sliding fee scale.
  • We are a National Health Service Corp site, and our providers are eligible to apply toward a federal loan forgiveness program
